Tesamorelin is a stabilised 44-amino-acid GHRH analogue, FDA-approved for HIV-associated lipodystrophy. It binds pituitary GHRH receptors to drive physiological pulsatile GH secretion, restoring IGF-1 levels without disrupting feedback regulation. Trials show a ~15–20% reduction in visceral adipose tissue measured by CT after 26 weeks, along with improvements in triglycerides, adiponectin, and cognitive markers, while preserving subcutaneous fat and insulin sensitivity.
